Why do I need to replace my Ford Transit Custom's fuel filter regularly?

I have a Ford Transit Custom and have noticed that the engine has been running rough and sluggish lately. I heard from a friend that it could be because of a dirty fuel filter. Is this true? And if so, how often should I replace the fuel filter to keep my van running smoothly?
