Why is my Ford Galaxy experiencing transmission issues?

I have a 2015 Ford Galaxy with 80,000 miles on it and recently it has been having trouble shifting gears. I noticed a delay when accelerating and it sometimes jolts when shifting. What could be causing this issue?
