Why does my Ford Focus make a loud clunking noise when shifting gears?

I recently bought a used Ford Focus and I've noticed that it makes a loud clunking noise whenever I shift gears. I'm concerned that this could be a sign of a larger problem. What could be causing this and how can I fix it?
