Why is the transmission on my Ford Mondeo jerking when shifting gears?

I've noticed that when I shift gears on my Ford Mondeo, the transmission sometimes jerks and has a slight delay. This is a newer car, so I'm not sure what could be causing this issue. Is this normal for this model or is there an underlying issue I should be concerned about?
